All Cheerleaders will need the following paperwork.

All of our Registration forms and payments will be processed through this website but  these will need to be printed, completed and signed before your Cheerleader can participate on August 1, 2023.  This is mandatory to begin practices. 

All below items/forms  need to be completed and turned into the Cheer Director/Cheer Coordinator/Team Parent by August 1st.

Pop Warner Medical History Form (below part 1 and part 2)
Pop Warner Participant Contract (below) child's signature required
Original Birth Certificate (certified copy with raised seal)
Report card showing grades for full year
Hawks Parental Responsibility Form (below)
Hawks Media Permission Form
